JOB PURPOSE
The Activities Co-ordinator works as part of the multidisciplinary team carrying out a range of activities identified from the individual care plan and “What matters to me” which will stimulate and engage the patient in non-care related activities. They may undertake clinical care tasks and other duties as delegated by and in support of a Registered Nurse.
Support and supervision is provided by the Registered Nurse.
In partnership with the Registered Nurse the Activities Co-ordinator will review the patient’s individual care plan and will competently perform the necessary aspects of holistic and personal care, within their scope of clinical practise, for the patient as per the individual care plan to a high standard, recording care delivery and reporting back to the Registered Nurse.
Responsibilities- Work as part of the multidisciplinary team to deliver activities identified from the individual care plan and the patient’s preferences.
- Undertake clinical care tasks and other duties as delegated by and in support of a Registered Nurse.
- Record care delivery and report back to the Registered Nurse to ensure continuity of care.
- Review and contribute to the patient’s individual care plan in collaboration with the Registered Nurse.
- Ability to work within a multidisciplinary team and to support patients with non-care related activities as part of the care plan.
- Understanding of holistic and personal care within scope of clinical practice.
- PVG Scheme membership/record (or the ability to obtain) for regulatory groups as required (child and/or adult).
- Knowledge of or willingness to learn about criminal record checks and changes affecting PVG requirements as of 1 April 2025 and onwards.
- Clear information about eligibility to work in the United Kingdom.
