We’re currently hiring for a Case Manager in our Children’s Residential program!

$22.00/hr – $22.00/hr

The Case Manager plays a key role in supporting the day-to-day operations of assigned programs and ensures that youth have access to the community services they need. This position involves participating in an on-call phone rotation, which may require working directly in the homes to provide Direct Care support when needed. Professional communication and documentation skills are essential, as the Case Manager serves as a primary contact for youth, families, and community partners.

The role requires strong organizational and coordination abilities to ensure quality care and safety for youth in the program. This position also includes networking with external organizations to help achieve the agency’s strategic goals. The Case Manager must consistently model agency standards of care and conduct while working both independently and as part of a team.

• Full-time career available – 40 hours per week.
• Work a variable and flexible schedule, including evenings and weekends, to meet the needs of the children and the program.
• Participate in a week-long on-call rotation every four weeks.
• Pay rate of $22 per hour.

• Develop and deliver specialized behavior programs and Individual Service Plans (ISP) for youth, including completing ISP documentation, discharge reports, and coordinating and conducting ISP meetings.
• Serve as a primary contact for stakeholders and external providers, ensuring timely submission of documents and coordinating all court‑mandated visitations and protocols.
• Oversee medication management, including scheduling appointments and reporting any medication errors.
• Design, supervise, implement, and track individualized life‑skills training programs for youth while compiling data for Outcome Management and Quality Assurance.
• Represent youth in court, family involvement meetings, school meetings, and other sessions with lead agencies and stakeholders.
• Screen and interview admission referrals and potential new youth placements.
• Ensure all reportable incidents are promptly investigated, documented, and followed up with appropriate support for youth and stakeholders.
• Participate in administrative team meetings and agency committees, including the Joint Leadership monthly meeting.
• Assist in managing department activities, such as recruiting, hiring, handling employee relations, and providing orientation, supervision, and training to direct support staff in line with agency policies.
• Develop and maintain a positive community presence by fostering networking opportunities, enhancing youth services, and increasing community awareness of the agency’s mission.
• Ensure residential homes and agency vehicles are properly maintained and staffed. Support compliance with DDA and DHS OLM licensing requirements, including policy reviews, re‑licensing applications, CJAMS reports, and Corrective Action Plans. Conduct annual strengths and needs assessments for each youth to guide individualized service planning.

• Bachelor’s degree with a minimum of two (2) years of experience working with adolescents or young adults, OR an associate’s degree with a minimum of three (3) years of experience.
• Must be 21 years of age or older.
• Must demonstrate the ability to establish and maintain professional boundaries with youth.
• Prior experience in behavioral management, including implementing strategies to promote positive behaviors and effectively addressing challenging situations.
