Physical Therapist (PT) Home Health - Full Time or Part Time
Location: Half Moon Bay, CA
ABL Health Care and Hospice is a trusted provider of home healthcare and hospice services. We are seeking a compassionate, organized, and team-oriented Physical Therapist to join our Home Health team as either Full‑Time or Part‑Time in Half Moon Bay, CA.
Position Overview
We are looking for an experienced, self‑driven PT to provide exceptional care throughout the region. You will evaluate patients’ physical health, develop individualized treatment plans, and work to improve mobility, strength, and overall quality of life. Collaboration with the Clinical Director and Clinical Staff will ensure the highest standards of care while supporting both remote and independent field work.

- Provide physical therapy services within scope of care, following individualized plan.
- Administer treatments, monitor patient conditions, and report changes to Physician, Case Manager, or Director of Nursing as needed.
- Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
- Comply with all company policies and procedures to ensure high standards of care.
- Current PT license in California.
- At least 1 year experience in Home Health or Hospice care.
- Current California driver’s license.
- Able to travel within a 15‑25 mile radius regularly.
