Registered Nurse (RN) Home Health – Full Time / Part Time (Per Diem)

We are seeking an experienced, self‑driven RN to join our dedicated Home Health team and provide exceptional care throughout the region. You will evaluate patients’ health, create and implement care plans, track their progress, and collaborate with our Clinical Director and Clinical Staff both remotely and independently in the field.

• Provide nursing care within the scope of practice, following individualized care plans and coordinating with the interdisciplinary team.
• Administer treatments, monitor patient conditions, and report changes to Physician, Case Manager or Director of Nursing as needed.
• Maintain accurate clinical records and submit documentation per company policy using a company‑provided device.
• Supervise Licensed Vocational Nurses (LVNs) when assigned.
• Comply with all company policies and procedures to ensure high standards of care.

• Current Registered Nurse (RN) license to practice in California.
• Minimum 1 year of experience in Home Health or Hospice care.
• Current California Driver’s License.
• Ability to travel within a 15‑25 mile radius regularly.
