Position: Mid-Level LPN Role at Poseidon Care Centre

Reporting to the Director of Care or Health & Wellness, you will be responsible for managing resident care in accordance with provincial standards. Tasks include thorough clinical assessments, implementing care/service plans, and effectively communicating with both residents and families. Collaboration with various healthcare professionals is essential to ensure the best outcomes.

Key Responsibilities:

• Develop and manage individualized care plans for residents

• Document care accurately and facilitate resident education

• Participate in care conferences to enhance treatment discussions

• Oversee medication administration and compliance

• Address resident and family concerns proactively

Requirements:

• Valid registration as a Licensed Practical Nurse

• Experience in geriatrics or long-term care settings

• Strong time management and coordination skills

• Current BCLS and First Aid certifications required

• Proficiency in Microsoft Office applications
