At the South West Yorkshire Partnership NHS Trust, we seek a dedicated full-time practitioner for our home-based treatment services. This position emphasizes nurturing relationships while conducting assessments and planning interventions for clients in distress. You’ll play a vital role in a flexible, integrated team focused on providing high-quality care.
Key Responsibilities:
• Assess client needs and develop tailored care plans
• Implement interventions to support mental health recovery
• Work collaboratively in a multi-disciplinary environment
• Provide interventions during discharge planning
• Engage in service governance and improvement activities
Requirements:
• Current UK registration as a Mental Health Nurse or equivalent
• Proven experience in mental health assessment and care
• Strong communication skills and independent working capability
• Availability for flexible 12-hour shifts
• Competency in IT systems for care management
