As a Client Service Assistant in the CC Continuing Care Coordination department, you are vital to the successful delivery of Continuing Care programs. This permanent full-time role allows you to work closely with clients, elucidating program requirements and aiding them in completing necessary documentation. Your collaborative spirit and problem-solving skills will shine as you support both clients and fellow team members in ensuring holistic care.
Key Responsibilities:
• Assist clients in completing Continuing Care forms
• Support marginalized clients in navigating documentation
• Share community resources with the care team
• Act as liaison for escalated services within the team
• Respond to client inquiries regarding services
Requirements:
• Completed two-year college program in health services
• At least one year of relevant experience
• Current Emergency First Aid and CPR Certification
• Intermediate computer skills in Microsoft Office
• Access to a reliable vehicle for client visitations
