Be a vital support for individuals and families as a Patient Navigator with Kwanlin Dün First Nation at the Natsékhi Kų̀ Health Centre. Facilitate access to healthcare services and empower clients to navigate their health journeys.
This role requires a Bachelor of Social Work Degree and at least two years of relevant experience. You will advocate for clients, employing skills in crisis intervention, psycho-social assessment, and navigation of complex care needs. Collaboration with community members and healthcare professionals is essential to enhance well-being and deliver integrated care solutions.
Key Responsibilities:
• Assist clients in accessing healthcare resources and services
• Coordinate comprehensive wellness and care planning
• Build strong relationships with families and community stakeholders
• Provide emotional and practical support to clients
• Enhance health literacy among clients and families
Requirements:
• Bachelor of Social Work Degree with 2+ years’ experience
• Experience in counseling and crisis intervention
• Knowledge of complex care needs and advocacy
• Familiarity with Kwanlin Dün community services
• Valid Class 5 Driver's License and TB Clearance
