Case Manager for STBBI Prevention

In this full-time position until March 2027, you'll be responsible for conducting assessments, managing wellness checks, and facilitating access to harm reduction supplies within the Addiction Medicine Clinic. Collaborating with healthcare providers, you will play a key role in community health education and support.
Key Responsibilities:
• Facilitate intake and treatment planning for clients
• Manage Wellness Recovery Programs for addiction
• Deliver health promotion through educational sessions
• Conduct wellness checks via the Mobile Clinic Van
• Assist with Contact Tracing initiatives for STBBIs
Requirements:
• Diploma in Health or Social Services required
• Two years of relevant experience with Mental Health
• CPR and First Aid certification necessary
• Training in Crisis Intervention and De-escalation
• Knowledge of Anishnaawbe culture preferred
