In this role, you will create holistic care strategies for patients transitioning from hospital to home. As part of the interdisciplinary SE Transitions team, you’ll focus on client’s physical, emotional, social and spiritual needs. Candidates should have experience in community rehabilitation, alongside strong communication and organizational skills.

Key Responsibilities:

• Conduct detailed patient assessments collaboratively

• Formulate personalized goals and treatment recommendations

• Plan goal-directed treatment programs effectively

• Work alongside rehab assistants and a high-performance team

• Maintain strong knowledge of community resources

Requirements:

• Experience in acute or community-based rehabilitation preferred

• Valid driver's license and vehicle for travel

• Flexible, self-directed work ethic

• Strong interpersonal and organizational skills

• Positive work record and attendance essential
