As a Nurse Practitioner, you will play an integral role in supporting the patient rehabilitation, recovery and reintegration from the facility to their own home. There’s a chance to participate in program development and system leadership whilst the program grows.
You will create evidence-based care plans and work in a multidisciplinary team which includes Nursing, Social Work and Rehabilitation professionals.

Responsibilities:
Practice independently by assessing patients, ordering and interpreting diagnostic investigations, and determining appropriate next steps
Prescribe medications and carry out clinical procedures in accordance with your authorized scope of practice
Work in collaboration with physicians and multidisciplinary care teams to ensure safe, effective transitions from hospital to home
Engage patients, families, and interdisciplinary partners to develop individualized care plans that reflect patient goals and preferences
Support continuous improvement and innovation by contributing to quality initiatives within transitional health care programs
Requirements:
A Master’s degree in Nursing with Nurse Practitioner Certification
registration with the Nova Scotia College of Nursing in good standing
Experience supporting vulnerable or complex populations through admission, care planning, or discharge (an asset)
A passion for helping patients safely return home and maintain independence.
Fluency in English
