You will work in a transitional care setting, managing 22–25 patients who are ready to move from acute care. This role involves close collaboration with interdisciplinary teams to facilitate successful discharge planning and promote patient independence.
Key Responsibilities:
• Manage post-acute inpatient care for adult patients
• Establish discharge goals within 24–48 hours
• Communicate effectively with families and care professionals
• Diagnose/manage various health conditions
• Collaborate with pharmacy partners on treatment plans
Requirements:
• Current license to practice in Nova Scotia
• Experience in transitional or post-acute care
• Full-time commitment for contract duration
• Thrive in interdisciplinary team environments
• Passionate about enhancing patient wellbeing
Contribute to a meaningful transitional care model that prioritizes effective discharge processes and promotes patient autonomy.
