As part of a multidisciplinary team, you will perform case management assessments, design individual care plans, and monitor client progress. Your clinical expertise and case management experience will play a crucial role in our clients’ health journeys.
Key Responsibilities:
• Complete in-depth case management assessments
• Develop comprehensive and individualized care plans
• Monitor and evaluate client health status
• Work collaboratively in a primary health care team
• Recommend and implement necessary adjustments to care plans
Requirements:
• Baccalaureate Degree in Nursing from an accredited institution
• 3 years of clinical nursing experience within the last 4 years
• Valid nursing license from your province
• Basic Life Support (BLS) certification required
• Prior experience in case management is necessary
