RN Case Manager for Bayshore

Provide exemplary nursing care as an RN Case Manager with Bayshore Medical Personnel in Halifax, NS. This full-time role is dedicated to supporting clients with protracted health challenges in a clinic setting.

As part of a multidisciplinary team, you will perform case management assessments, design individual care plans, and monitor client progress. Your clinical expertise and case management experience will play a crucial role in our clients’ health journeys.

Key Responsibilities:

• Complete in-depth case management assessments

Requirements:

• Baccalaureate Degree in Nursing from an accredited institution

• 3 years of clinical nursing experience within the last 4 years

• Valid nursing license from your province

• Basic Life Support (BLS) certification required
