Extendicare is seeking dedicated LPNs/RNs to care for patients in their homes across various settings. You'll assess individual care needs using professional tools, execute care plans, and ensure a comfortable environment for patients of all ages. This role requires flexibility and travel, providing an essential service to the community.
Key Responsibilities:
• Conduct home visits to attend to patient needs
• Provide ethical and safe nursing practices
• Use appropriate assessment methods for patient evaluation
• Monitor and adapt care plans as necessary
• Build rapport and trust with patients through engagement
Requirements:
• Must hold NSCN registration as an LPN or RN
• A valid driver’s license and vehicle required
• Previous community care experience preferred
• Flexibility and reliability in scheduling
• Willingness to participate in on-call patient coverage
