About The Opportunity
Assessing, planning, evaluating, documenting, and directing residents’ needs through consultation with medical professionals, care conferences and team members.
Providing leadership to health services staff and overseeing provision of quality health services respecting the client’s right to individualized planning.
Providing leadership and oversight to LPN and CCA activities to ensure compliance with policies, procedures, and applicable legislation.
Ensuring new team members are oriented and trained on all departmental standards.
Monitoring and managing performance and providing coaching.
Completing performance and development discussions for team members.
In consultation with HRBP and supervisor, ensuring disciplinary procedures and documentation are completed according to company policy.
Ensuring all work is performed safely in accordance with Occupational Health and Safety procedures.
Ensuring resident admissions/re-admissions are completed within the timeframe and according to practice.
Serving as a resource person for other nursing staff in identifying and solving nursing or resident care problems.
Delegating clinical responsibilities to qualified staff in accordance with policy and standards of practice.
Representing the Home at community and professional meetings; attending workshops, in-service sessions and other scheduled meetings authorized by the Site Manager/Regional Manager/General Manager.
Endorsing and implementing best practice guidelines, policies and procedures within the facility.
Assessing and reassessing resident health status and communicating resident care needs and changes in status to key contacts.
Ensuring resident plan of care is established and implemented in accordance with policy and that the care plan is developed collaboratively with the multidisciplinary care team.
Encouraging active participation by residents in therapy programs and recreational activities.
Developing strong relationships with residents, families, substitute decision makers, physicians, other care providers and the rest of the care team.
Ensuring regular ongoing communication with residents and families providing updates as part of the assessment process.
Working with existing management and health services teams to promote communication and compliance with policies and procedures.
Assisting with the development, implementation, and evaluation of resident care plans.
Administering and recording prescribed medication as per company policy and scope of practice.
Applying dressings and treatments according to physician’s orders and policy.
