Position: Full-Time Primary Health Care Nurse
Become a critical care provider as a Full-Time Primary Health Care Nurse in Moosomin. Support community health initiatives and deliver comprehensive nursing services at Southeast Integrated Care Centre.

This temporary nursing position at the Southeast Integrated Care Centre requires a collaborative spirit and a Bachelor's degree in Nursing. You will provide focused health care and participate in community development. Ideal candidates will have nursing experience and understanding of chronic disease management for optimal patient outcomes.

Key Responsibilities:

• Provide nursing services to patients in PHC settings

• Coordinate care while adhering to established standards

• Participate in community development initiatives

• Evaluate Primary Health Care initiatives with the team

• Support chronic disease prevention efforts

Requirements:

• Bachelor’s Degree or Diploma in Nursing required

• 2 years nursing experience in community health preferred

• Basic Life Support Certificate (BLS) required

• Experience in case management is an asset

• Strong facilitation and leadership skills needed
