The Opportunity
Reporting to the Manager of Hematology/Oncology/Nephrology and PACT, the Family Care Coordinator Registered Nurse (FCC RN) is a member of an interdisciplinary care team, providing holistic, comprehensive evidenced based care to meet the needs of Nephrology patients and families. The FCC RN provides leadership in nursing assessment and the planning, implementation, and evaluation of patient/family care. The FCC RN provides care in collaboration with the patient and family, other nurses, and health team members within the IWK and external organizations throughout all stages of health and complexities of illness.
The Nephrology FCC RN provides patient care within the Nephrology Team both in joint appointments with physicians and nursing-only visits. The FCC RN supports the coordination of care for patients followed within the Nephrology Clinic, the Renal Dialysis Unit, and 6

Link. The FCC RN will provide care and teaching for hemodialysis, renal transplants, PD, and other nephrology conditions as needed. The FCC RN will be responsible for monitoring the patient’s ongoing needs as well as completing assessments in person and virtually (including phone follow-up). The FCC will maintain competencies in all areas of nephrology pediatric patient care and will provide hemodialysis treatments and coverage.
