Family Care Coordinator Registered Nurse

This permanent full-time role involves leading comprehensive care assessments and supporting Nephrology patients and families at IWK Health. As a FCC RN, you will provide ongoing care through various methods, including virtual follow-ups and in-person assessments. Your responsibilities will include education on treatment methods such as hemodialysis and fostering collaboration among health teams.

Key Responsibilities:

• Lead patient assessments and develop care plans

• Provide education on nephrology treatments

• Collaborate with multidisciplinary healthcare teams

• Conduct follow-ups in person and virtually

• Maintain competencies in pediatric nephrology care

Requirements:

• Bachelor’s degree in Nursing from an accredited program

• Licensed or eligible for licensure in Nova Scotia

• Minimum of three years experience in pediatric nephrology

• Competency in Hemodialysis, Apheresis, and PD

• Strong organization and communication skills
