Bilingual Field Nurse Case Manager

Support patients across Atlantic Canada and Quebec as a bilingual Field Nurse Case Manager with clinical monitoring and patient education. This hybrid position requires regional travel up to 75%.
At Sentrex Health Solutions, the Field Nurse Case Manager coordinates Patient Support Program services for patients, physicians, and healthcare providers. You will manage registration, enrolment, follow-up, therapy support, and communication with healthcare partners. The work includes patient and provider education, case management protocols, data entry, and adverse event reporting.
Key Responsibilities:- Coordinate patient support services according to program protocols
- Register patients and healthcare providers, completing required follow-ups
- Provide education on diseases, therapies, and disease management
- Monitor adherence, therapy changes, clinical information, and adverse events
- Liaise with physicians, pharmacies, and infusion service providers
- Registered Nurse in good standing with the provincial nursing college
- Two to four years of clinical experience
- Bilingual English and French communication skills
- Experience in oncology, gastroenterology, or rheumatology
- Patient Support Program experience is an asset
