Be part of the Housing First Initiative with the North End Community Health Centre in Halifax as a Case Manager. Your primary task will be to guide individuals from homelessness into stable, permanent housing through personalized support and recovery planning.
This position requires managing a caseload of 15-30 participants, providing consistent support and developing customized housing plans. Expected to have two years of relevant experience, strong communication, and an understanding of harm reduction techniques.
Key Responsibilities:
• Develop recovery plans grounded in Housing First strategies
• Support individuals in securing housing and applications
• Advocate and connect clients to necessary resources
• Document client interactions accurately and promptly
• Collaborate with community and service agencies
Requirements:
• Post-secondary education in a related field preferred
• Two years’ experience working with populations at risk
• Excellent communication and documentation skills
• Familiarity with Housing First and Harm Reduction
• Commitment to confidentiality and professional standards
