This leadership position involves overseeing the daily operations of our social work and behavioral health teams in the PACE model. You will champion person-centered care by ensuring that participants' needs remain prioritized in service delivery. Collaborating closely with the interdisciplinary team, you will enhance care outcomes through effective management and communication.
Key Responsibilities:
• Lead and supervise social work and behavioral health teams
• Guide direct reports through coaching and development
• Foster participant engagement and positive PACE transitions
• Manage complex client issues as primary escalation point
• Assess needs and drive care planning resolutions
Requirements:
• MSW or equivalent experience in social work
• 2+ years leading participant-facing teams
• Knowledge of community-based services
• Strong collaborative and problem-resolution skills
• Valid California driver’s license and insurance
