This position with Anchor Youth Space embraces equity and is intended for members of the African Nova Scotian community. You will apply a relational approach, diagnosing and addressing needs of youth aged 12 to 25.
Your role is pivotal in providing thorough access to mental health services and supporting young people's personal growth.
Key Responsibilities:
• Guide youth on their mental and physical health journey
• Ensure seamless referrals to integrated support services
• Collaborate on individualized care plan development
• Help youth transition effectively to adult services
• Develop group programs fostering social connections
