Position: Case Manager for Housing First Program
Join North End Community Health Centre in Halifax as a Case Manager dedicated to the Housing First program. Support individuals facing homelessness through personalized case management and collaborative community engagement.

This position is specifically designated for 2

STGD++, African Nova Scotians, and Indigenous individuals. The Case Manager plays a vital role, managing a caseload of up to 30 participants, and facilitates monthly tenant meetings, crisis response, and ongoing support. Your focus will be on developing strength-based recovery plans that adhere to Housing First and Harm Reduction principles.

Key Responsibilities:

• Conduct tenant selection via BNL list with community partners

• Facilitate monthly tenant meetings for engagement

• Provide crisis response and ongoing participant support

• Develop strength-based recovery plans for clients

• Advocate for participants and connect with community resources

Requirements:

• Post-secondary education in human services or related fields

• 5+ years of experience with individuals facing homelessness

• Expertise in crisis management and participant advocacy

• Strong background in community service collaboration

• Proven commitment to confidentiality and professional standards
