STGD++, African Nova Scotians, and Indigenous people. As a Case Manager, you will oversee a caseload of 15-30 participants, facilitating essential resources, crisis response, and recovery plans grounded in Housing First principles. Your engagement with community partners is crucial for successful tenant support.
Key Responsibilities:
• Manage participant selection and tenant engagement initiatives
• Deliver ongoing crisis response and support services
• Maintain confidential documentation in accordance with standards
• Coordinate housing searches and rental applications for clients
• Foster relationships with service providers and stakeholders
Requirements:
• Relevant education in psychology, social work, or human services
• 5+ years working with homeless individuals
• Strong skills in case management and community outreach
• Experience in crisis intervention and support
• Ability to work with diverse populations and maintain confidentiality
Utilize your case management expertise to improve housing outcomes for vulnerable communities.
