Community Health Case Manager

STGD++, African Nova Scotians, or Indigenous. As a Case Manager, you will manage 15-30 participants, aiding in their housing searches and coordinating essential services. You will also implement recovery plans and facilitate tenant meetings, ensuring ongoing participant support.

Key Responsibilities:

• Facilitate the tenant selection and engagement process

• Provide health-focused case management support directly

• Assist with monthly meetings and community-building initiatives

• Maintain accurate and confidential participant documentation

• Collaborate with healthcare professionals and community services

Requirements:

• Post-secondary education in relevant fields or equivalent experience

• Over 5 years of related experience with homeless individuals

• Proven ability in crisis management and effective advocacy

• Background working within community health frameworks

• Commitment to organizational values and participant confidentiality
