The Stroke Early Supported Discharge Team (Stroke ESD) facilitates earlier discharge from hospital for stroke survivors, ensuring individuals receive the appropriate intensity of rehabilitation for up to 6 weeks following discharge home from hospital. The pathway consists of Occupational Therapists, Speech and Language Therapists, Physiotherapists, Specialist Stroke Nurses and Rehabilitation Assistants. There are close links to the Stroke wards at Calderdale Royal Hospital, the Community Rehabilitation team, the local Stroke Association, the Reablement service and exercise on prescription schemes.
The post holder will work within the Stroke ESD pathway providing Specialist Occupational Therapy assessment and rehabilitation for patients identified as being suitable for the service.
The majority of clinical work will take place within the patient’s home or temporary residence. However, the post holder will be expected to in‑reach to the hospital to help to identify patients who may be suitable for the Stroke ESD pathway, attend multi‑disciplinary team (MDT) meetings on the stroke wards, and to meet potential patients to start goal‑setting and care‑planning prior to discharge home.

Qualifications / Training- Registered with HCPC
- Evidence of completing Clinical Educator training
- Evidence of post‑graduate learning and study relevant to stroke
- Recent experience demonstrating ability to assess, treat and evaluate care following stroke.
- Comprehensive knowledge of appropriate and available Occupational assessments and approaches in the specialty of stroke, including outcome measures
- Experience of working as an Occupational Therapist in a variety of NHS settings including community working
- Knowledge of relevant legislation, national guidelines and best‑practice regarding stroke
- Proven ability in implementing Clinical Governance
- Critical Appraisal Skills
- Ability to maintain legible and accurate records
- Experience of case‑management
- Previous experience at Band 6 level.
- Teaching/Presentation skills
- Experience in developing, supervising and training others
- Ability to communicate effectively with people with complex difficulties and in difficult situations
- Ability to communicate with all stakeholders across the stroke pathway
- Recognise opportunities for learning and development both personally and within the broader team
- Contribute effectively to the multi‑disciplinary team
- Ability to work in a stressful environment and concentrate in conditions requiring emotional demands
- To be flexible and adaptable within the working day
- To have excellent time management and organisational skills
- Ability to organise own work and caseload
