We have a brilliant opportunity for an Adults Team Leader to join the Hospital Discharge and Post Discharge Assessment Team at Calderdale Council, based in Princess Buildings, Halifax (HX1 1TS
). This temporary contract runs from 30th June 2026 to 30th October 2026, offering a competitive rate of £40 per hour (Umbrella/PAYE) for full-time work at 36.2 hours per week.

As an Adults Team Leader, you will have a vital role in leading and supporting the team to ensure efficient and effective hospital discharges and post-discharge assessments. Your expertise will be instrumental in enhancing patient care and safety in the community.

What you will do:
  • Lead and manage the Hospital Discharge and Post Discharge Assessment Team to ensure timely and safe discharges from hospital settings.
  • Oversee assessments and care planning activities, ensuring they are carried out to high standards and comply with relevant legislation and guidelines.
  • Provide expert guidance and support to team members, assisting them with complex cases and fostering a supportive team environment.
  • Collaborate with multidisciplinary stakeholders to ensure integrated service delivery and improve outcomes for adults leaving hospital care.
  • Monitor the team's performance, implement improvement strategies, and report outcomes to senior management.
Requirements of the Team Leader:
  • Degree or equivalent in social work with current Social Work England registration.
  • Significant frontline experience in Social Services within the UK as a qualified Social Worker.
