We are looking for a creative and resourceful Social Care Service Co-ordinator to work with us in the LinC into Calderdale Team which is part of the Adult Services and Wellbeing Directorate at Calderdale Council. The team provides guidance and assistance to people who may not meet the Care Act eligibility but who, nevertheless, requires some support and to prevent, reduce or delay the need for stator services.
We work in partnership with a variety of community-based organisations, Health and Public Health partners, to identify and direct people to appropriate place-based service to enable people to live a fulfilled life.
We use strengths-based practice focussing on positive outcomes and being independent and strive to put the person at the heart of assessments.

The Social Care Service Co-ordinator position is a valued and vital role that specialises in working with adults who have additional requirements to live their largest life. The team is fast paced, and we support people with complex needs and the ability to assess risk and manage risk is vital. We challenge misconceptions and inequalities within our role to ensure we respect the persons voice and wishes.
Whatwe are looking for:
- Ability to relate to people Your role as a Social Care Service Co-ordinator is to have direct contact with people, usually on a face-to-face basis. To join our existing team, you will have an expert ability to quickly develop a trusting relationship with people, utilising a kind, empathetic and friendly approach. You must be able to not only listen deeply to what people are telling you but also be skilled at adapting your own communication to ensure you get the best out of the interactions you have with the people who call upon our support.
- Resourcefulness and creativity You will recognise that formal services are not always solutions. You will need to be useful, resourceful, and creative when it comes to drawing on the strengths of our people, their wider networks, as well as your local knowledge of the community assets available to problem solve and meet their requirements. You will need to be confident to challenge the status quo or preconceived ideas to enable this happen.
- Adult Social Care Knowledge The type of person who will do well in this role will have experience and knowledge of adult social care. You may have direct lived or living experience of adult social care yourself, or maybe involved or close to someone who has. You may be a care worker or in a similar role and looking for your next career move.
