Physical Therapist (PT) – Skilled Nursing Facility | Hamden, CT

Med Adventures is seeking an experienced Physical Therapist to join a skilled nursing facility in Hamden, CT. In this role, you’ll help residents regain mobility, strength, and independence while working within a supportive, collaborative rehab team.

• Active Connecticut PT license
• SNF or geriatric rehab experience preferred
• Strong clinical reasoning, communication, and teamwork skills
• Commitment to high-quality, resident-centered care
