Outpatient Clinical Pharmacist – Smilow Cancer Hospital Overview

Responsibilities- Customer Service Quality – Provide excellent customer service, medication counseling, and effective communication. Demonstrate CHAMP behaviors and elevate issues as appropriate.
- Medication Management – Conduct timely medication order review, medication therapy monitoring, and collaborate with the healthcare team. Adjust therapy and ensure clinical continuity where applicable.
- Medication Preparation & Distribution – Competency in pharmacy automation, sterile and non-sterile compounding, medication distribution, and controlled substance management.
- Medication Safety, Regulatory Affairs & Quality Assurance – Adhere to departmental and state/federal regulations, report medication events, apply HRO principles, and participate in quality assurance activities.
- Leadership – Provide education and training, participate in professional development, attend staff meetings, and oversee pharmacy staff as necessary.
- Financial/Volume Results – Contribute to cost reduction and revenue generation, participate in pharmacist-driven cost management protocols, and provide cost-effective clinical care.
- Immunization Training – Successful completion of an immunization training program compliant with state regulations.
- Education – Graduation from an accredited College of Pharmacy with a B.S. or Pharm
D degree. - Experience – Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable.
- Licensure – CT Pharmacist Licensure required, active and without restrictions. BLS and immunization certification required within 6 months of hire.
- Special Skills – Basic knowledge of pharmacology and therapeutics; understanding of pharmacy systems for drug distribution and formulary management; excellent verbal and written communication; expertise in patient management.
- Physical Demands – Travel to care sites for meetings may be required.
