Responsibilities- Customer Service Quality:
Demonstrate and promote CHAMP behaviors;
Provide excellent customer service to patients and other healthcare team members;
Provide medication counseling to patients and families;
Demonstrate effective oral and written communication skills;
Demonstrate ability to elevate issues to management or collaborating prescribers as appropriate. - Medication Management:
Conduct timely, accurate medication order review that considers all patient characteristics including laboratory values and vital signs;
Provide medication therapy monitoring for assigned patients, adjusting therapy based on patient factors;
Develop an effective interdisciplinary relationship with the health care team;
Act as a source of medication information for the health care team;
Participate in patient-centered initiatives;
Ensure clinical continuity where applicable to work area. - Medication Preparation and Distribution:
Demonstrate competency in pharmacy automation;
Oversee sterile and non-sterile compounding to ensure compliance with regulatory statutes;
Oversee medication distribution/dispensing process in work area, ensuring safe, effective, timely medication distribution/dispensing;
Demonstrate competency in and oversee controlled substance management, including monitoring and documentation. - Medication Safety, Regulatory Affairs, and Quality Assurance:
Adhere to all departmental policies, procedures, protocols, and guidelines;
Adhere to appropriate state and federal regulations;
Report medication events and near misses;
Demonstrate HRO principles and conduct in all interactions with patients and staff;
Participate in quality assurance activities. - Leadership:
Provide education and training to other pharmacy staff members, pharmacy students and residents, and health care team members;
Participate in professional development activities;
Attend departmental staff meetings;
Oversee pharmacy staff as necessary in work area. - Financial/Volume Results:
Contribute to departmental cost-reduction/revenue generation;
Participate in pharmacist-driven cost management protocols where applicable;
Provide cost-effective clinical care. - Immunization:
The pharmacist has successfully completed an immunization training program that complies with the requirements of section 20-633-3 and section 20-633-4 of the Regulations of Connecticut State Agencies.
Graduation from an accredited College of Pharmacy with a B.S. or Pharm
D degree
Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required.
LicensureCT Pharmacist Licensure required, active and without restrictions. If required, will be responsible for obtaining out of state licenses to register for non-resident pharmacy licenses. BLS (Basic Life Support) certified or obtained within 6 months of hire. Immunization certified or obtained within 6 months of hire.
Special SkillsThis individual should demonstrate basic knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required. Must have expertise in patient management.
Physical DemandTravel to care sites for meetings may be required.
