• Develop, coordinate, and oversee individualized care plans for homecare clients
• Conduct client assessments and regular check-ins to monitor health, safety, and well-being
• Serve as the primary point of contact for clients, families, and caregivers
• Supervise and support direct care staff to ensure high-quality service delivery
• Coordinate with healthcare providers, social workers, and community resources
• Maintain accurate client records and documentation in compliance with regulations
• Respond to client concerns, incidents, and changes in care needs in a timely manner

• Experience in homecare, case management, or a related healthcare field
• Strong knowledge of care planning, patient advocacy, and community resources
• Excellent communication and interpersonal skills
• Ability to manage multiple cases and prioritize effectively
• Compassionate, patient-centered approach to care coordination
• Proficiency with documentation and basic computer applications
• Valid driver's license and reliable transportation preferred
