Your Role as a Pediatric Registered Nurse – Case Manager:
As a Case Manager, you’ll play a vital role in coordinating and overseeing the care of pediatric patients while serving as a key advocate for families. Your responsibilities will include:
• Patient Assessments & Care Planning – Conduct comprehensive assessments, develop individualized care plans, and adjust treatments as needed.
• Care Coordination & Advocacy – Collaborate with physicians, therapists, and caregivers to ensure seamless, high-quality patient care.
• Family Education & Support – Teach families about treatment plans, disease management, and best practices for home care.
• Clinical Nursing Care – Provide direct care, including medication administration, wound care, tracheostomy/ventilator management, and emergency interventions.
• Monitoring & Documentation – Track patient progress, recognize changes in condition, and ensure accurate documentation within the client’s record.
• Leadership & Team Collaboration – Work effectively within a multidisciplinary team, demonstrating flexibility and problem-solving skills in various healthcare situations.
• Compliance & Quality Assurance – Uphold Elara Caring’s mission, philosophy, and policies to maintain the highest standards of care.
What You Need to Succeed:

Education:
High School Diploma or GED
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• Experience – At least 1+ year of RN experience with Pediatric experience
• Physical Requirements - Ability to sit, stand, bend, lift, and move intermittently, with the capability to liftlbs.
• Licensure – Active and unrestricted RN license in the state of practice.
• Transportation – Reliable vehicle, valid driver’s license, and current auto insurance (per state laws).
