Overview

Hamilton Continuing Care has an exciting opportunity for a Registered Practical Nurse to join their team.

The hiring rate for this role is $32.75 per hour per collective agreement/employee association agreement.

Responsibilities
  • Maintain current registration with the College of Nurses of Ontario.
  • Have existing Heart Save Level of CPR.
  • Demonstrate an ability to prioritize tasks effectively, properly plan for, and allocate the right amount of time and effort to complete tasks. Remains calm and manages stress appropriately.
  • Deliver safe administration of medication and treatments with a focus on accuracy, quality and meeting residents’ quality of life requests.
  • Follow plans of care (or service plans) in accordance with the standards of the College of Nurses and able to identify and report any changes in the resident’s well-being to the Charge Nurse or Village leadership team.
  • Have an understanding of relevant policies, procedures, legislation, and regulations applicable to the Long-Term Care sector, including the Fixing Long-Term Care Act, 2021, and the Occupational Health & Safety Act (OSHA) of Ontario.
  • Support and embrace resident-centred social culture and deliver the highest quality of personal care to residents.
  • Work as a key member of the Village and Neighbourhood multidisciplinary team to understand residents’ individual needs, advocate for residents to participate in decisions about their care, support new resident move-ins, facilitate transfers to hospital, or respond to changes in resident medical needs.
  • Collaborate with the Neighbourhood Physician during weekly rounds and update care plans and directions accordingly.
  • Demonstrate proficient communication and computer skills to update care plans in the current system.
  • Understand the physical, mental, and emotional demands of the RPN role.
  • Demonstrate alignment with the organization’s vision, mission, and values and the Customer Experience Credo (Know Me, Be Present, Walk in My Shoes, Earn Trust and Follow Through).
Qualifications
  • Maintain current registration with the College of Nurses of Ontario.
  • Heart Save CPR certification.
  • Ability to prioritize and manage time effectively; remain calm under pressure.
  • Commitment to resident safety and quality of life in long-term care.
  • Knowledge of relevant policies, procedures, legislation, and regulations applicable to the Long-Term Care sector (e.g., OSHA, LTC standards).
Assets
  • MDS credentials (asset).
  • Experience working with people living with Dementia (asset).
  • Knowledge of palliative assessment scales (asset).
  • Comfortable supporting residents and families during end-of-life care (asset).
  • Knowledge of policies, procedures, legislation and best practices in retirement and long-term care (e.g., RAI processes, CIHI, supplies, equipment ordering) (asset).
