Location:

Hamilton - Onsite Full-time - Permanent - 35 Hours per week Monday to Friday - Flexible working hours between 8:00am - 5:00pm Our client is seeking a Bilingual Nurse Practitioner (French/English) to join their primary care team in Hamilton. Reporting to the Primary Care Services Coordinator, you'll provide comprehensive, client-centered health care focused on health promotion, illness prevention, and chronic disease management.

You'll work both independently and collaboratively with physicians and other healthcare professionals to deliver quality care that supports the well-being of our community. What Our Client Offers:

Provide primary health care services to clients of all ages. Assess, diagnose, and manage common health conditions independently or with physician support. Engage clients in identifying health risks, setting goals, and choosing care plans. Prescribe and interpret diagnostic tests as needed. Maintain accurate, confidential client health records. Support clients with stable chronic conditions and ensure appropriate follow-up care. Refer clients to internal programs and community resources.

Participate in policy and procedure development, quality improvement, and data collection. Collaborate with an interdisciplinary team to ensure coordinated, holistic care.

Qualifications:

Bilingual (French & English) - spoken and written Bachelor of Science in Nursing and Nurse Practitioner Certificate from a recognized university. Good standing with the College of Nurses of Ontario (CNO). Excellent clinical judgment, problem-solving, and communication skills. Ability to work both independently and within a multidisciplinary team. Proficient in standard computer applications and electronic health records.
