Location:

Home-to-home in the Greater Hamilton and Burlington Area.
Pay:  Pay per visit.
Shift

Schedule:

Monday through Friday, Alternating Weekend Rotation: 7:00am–3:00pm; 8:00am–4:00pm; 8:30am–4:30pm; 3:00pm–11:00pm as assigned.
On‑Call Rotation:  11:00pm–7:00am.
Statutory Holiday Rotation:  Provided.
What You’ll Do  As a Visiting Nurse, you will deliver personalized high‑quality care directly in clients’ homes. You will act as a primary care planner, provide holistic care, and enhance your skills through training, mentorship, and preceptorship programs.
Participate in on‑call rotations as required (typically twice per month, overnights)
Conduct in‑home assessments and deliver nursing care based on individualized care plans
Administer medications, wound care, IV therapy, palliative, pediatric care, and other clinical interventions
Monitor patient progress and update care plans accordingly
Educate patients and caregivers on disease management, medication adherence, and health promotion
Collaborate with interdisciplinary teams and delegate tasks to ensure continuity of care
Maintain accurate and timely documentation using electronic health records

Education & Certification   Graduate of an accredited School of Nursing (RPN)
Current registration in good standing with the provincial regulatory body (CNO)
LEAP Training, Palliative Certification (considered an asset)
Experience   2–5 years of recent relevant nursing experience in a fast‑paced clinic or medical environment

Experience with wound care, diabetes management, trach/vent, IV care is an asset
Other Requirements   Valid G2 or G Driver’s License and reliable vehicle
Willingness to travel regularly within the Hamilton/Burlington area in all weather conditions
