Community Nurse - Visiting

We are looking for RNs and RPNs for full‑time/part‑time/casual positions in the Tobermory on and surrounding areas.
Hours are 8AM‑8PM on a 2‑week rotation that includes weekends, weekdays, and on‑call work.

- Registered Nurses (RN): hourly range $32.08 – $47.38, based on hours worked and role‑specific criteria
- Registered Practical Nurses (RPN): hourly range $27.17 – $33.37, based on hours worked and role‑specific criteria
- Developing and maintaining effective Care Plans within Community Health Services, independently or in cooperation with Ontario Health atHome
- Working with patients, both in‑person and virtually, to provide exceptional nursing services
- Providing services in a variety of settings such as in‑home, clinic, retirement home or school
- Communicating and liaising with patients, family members and others within the interdisciplinary care team
- Performing other clinical duties as required
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- A strong desire and commitment to making a difference in patients’ lives
- Exceptional interpersonal skills
- A valid driver’s license and vehicle, as regular travel is required
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record, including a two‑step TB test
- Current vulnerable sector check and a clear background check
In addition to providing home‑based health care, Care Partners serves the community through clinics, transitional care units and relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we have been organizing staff‑led medical care and clinics in countries with poor access to health care since 2009.
