This part-time, temporary role spans 14 months, based in the dynamic multidisciplinary team  a Primary Care Nurse, you will manage patient care from referral through the transplant journey, including education and support. Ideal candidates will have experience in nephrology or transplant nursing and effective communication skills.

Key Responsibilities:

• Coordinate patient care from initial assessment to post-transplant

• Provide education to patients and families on the transplant process

• Serve as the primary contact for families and healthcare providers

• Collaborate with partners like Trillium Gift of Life Network

• Participate in local initiatives improving transplant care

Requirements:

• Current registration with the College of Nurses of Ontario

• Minimum two years of nephrology, dialysis, or transplant nursing

• Leadership experience in related nursing roles

• Current BCLS/HCP certification

• Proficiency with Microsoft Office and patient information systems
