Job Details

Location: Home-to-home in the Greater Hamilton and Burlington Area.

Pay: Pay per visit.

Shift Schedule: Monday through Friday, Alternating Weekend Rotation: 7:00am–3:00pm; 8:00am–4:00pm; 8:30am–4:30pm; 3:00pm–11:00pm as assigned.

On‑Call Rotation: 11:00pm–7:00am.

Statutory Holiday Rotation: Provided.

What You’ll Do

As a Visiting Nurse, you will deliver personalized high‑quality care directly in clients’ homes. You will act as a primary care planner, provide holistic care, and enhance your skills through training, mentorship, and preceptorship programs.

  • Participate in on‑call rotations as required (typically twice per month, overnights)
  • Conduct in‑home assessments and deliver nursing care based on individualized care plans
  • Administer medications, wound care, IV therapy, palliative, pediatric care, and other clinical interventions
  • Monitor patient progress and update care plans accordingly
  • Educate patients and caregivers on disease management, medication adherence, and health promotion
  • Collaborate with interdisciplinary teams and delegate tasks to ensure continuity of care
  • Maintain accurate and timely documentation using electronic health records

Education & Certification
  • Graduate of an accredited School of Nursing (RPN)
  • Current registration in good standing with the provincial regulatory body (CNO)
  • LEAP Training, Palliative Certification (considered an asset)
Experience
  • 2–5 years of recent relevant nursing experience in a fast‑paced clinic or medical environment
  • Experience with wound care, diabetes management, trach/vent, IV care is an asset
Other Requirements
  • Valid G2 or G Driver’s License and reliable vehicle
  • Willingness to travel regularly within the Hamilton/Burlington area in all weather conditions
Vaccination Requirement
