Registered Practical Nurse

Hamilton Continuing Care seeks a dedicated Registered Practical Nurse to join its multidisciplinary team. You will maintain registration with the College of Nurses of Ontario and demonstrate effective task prioritization. Your responsibilities will include administering medications safely and adhering to care plans while supporting residents' physical and emotional needs.

Key Responsibilities:

• Maintain current registration with the College of Nurses of Ontario

• Administer medication accurately with quality focus

• Follow care plans and report changes in resident well-being

• Collaborate with Neighbourhood Physician during rounds

• Advocate for residents and assist during hospital transfers

Requirements:

• MDS credentials and experience with Dementia care

• Knowledge of palliative assessments and end-of-life support

• Familiarity with Long-Term Care policies and regulations

• Strong communication and computer skills

• Ability to manage emotional and physical demands of the role
