Nurse Practitioner

Position: Full-Time Nurse Practitioner Role
Join Hamilton Family Health Team as a full-time Nurse Practitioner at the Eva Rothwell Centre, where you will deliver essential primary care services. This role focuses on managing chronic diseases and offering preventive care for diverse populations.

You will engage with patients and collaborate with a multi-disciplinary team to ensure effective chronic condition management. Your commitment to equity-centered care and strong diagnostic skills will empower patients navigating complex health challenges.

Your role will extend to preventive care, including well-woman and prenatal services.

Key Responsibilities:

• Manage and treat complex chronic health conditions

• Provide well-woman and well-child preventive services

• Lead care coordination among team members and partners

• Document care in compliance with health regulations

• Support patients with social determinants of health

Requirements:

• Registered Nurse Practitioner in Extended Class with Ontario

• Master’s in Nursing or equivalent NP education

• Current CPR certification required

• Experience in trauma-informed primary care practices

• Familiarity with electronic medical records is an asset
