You will play a vital role in the healthcare team, with responsibilities ranging from conducting in-home assessments to managing ongoing patient care plans. Key Responsibilities:
• Conduct nursing assessments and administer treatments
• Monitor patient care and update plans based on progress
• Collaborate with healthcare teams for comprehensive care
• Educate clients and families about health and wellness
• Maintain detailed electronic health records of care provided
Requirements:
• RN graduate from an accredited nursing school
• Valid provincial registration with CNO
• 2-5 years nursing experience in a healthcare setting
• G2 or G driver’s license and reliable vehicle
• Willingness to travel for in-home visits in all weather
