The City of Hamilton is actively seeking a Nurse Practitioner for a full-time position at Macassa Lodge. This role emphasizes providing comprehensive primary health care focused on the unique needs of geriatric populations. Collaborate effectively within interdisciplinary teams to deliver optimal healthcare solutions, ensuring residents receive tailored support.

Key Responsibilities:

• Facilitate direct clinical care for residents in need

• Conduct detailed assessments and develop care strategies

• Liaise with interdisciplinary teams for cohesive resident care

• Oversee chronic illness management and reporting

• Promote optimal health through education and resources

Requirements:

• Baccalaureate in Nursing with current RN registration

• Experience or certification in Gerontological Nursing preferred

• Solid understanding of chronic disease and health management

• Excellent communication and teamwork skills required

• Must possess a valid Class “G” Driver's Licence
