Position: RPN with Community Engagement Focus
Join the Hamilton Family Health Team as a Registered Practical Nurse focused on community engagement. Support unattached patients in accessing primary care and collaborate with health providers.

This full-time position will have you traveling to various HFHT clinic sites to deliver comprehensive patient care.

Your role as an RPN encompasses outreach, clinical onboarding, and fostering relationships with community partners. Your work will be vital in addressing social vulnerabilities among patients and facilitating their connection to healthcare resources.

Key Responsibilities:

• Conduct proactive patient engagement and outreach

• Complete initial patient charts and health assessments

• Develop relationships with local community partners

• Support patient transitions to primary care

• Ensure adherence to documentation standards

Requirements:

• Registration in good standing with CNO as RPN

• Two years of relevant clinical nursing experience

• Familiarity with electronic medical records preferred

• Strong independent clinical assessment capabilities

• Experience with vulnerable patient populations
