Join Age Care as a part-time Nurse Practitioner, focusing on quality resident care. This collaborative position will allow you to make a difference in the lives of seniors within a supportive team environment.
In this two-day-a-week role, you will assess, diagnose, and implement treatment plans tailored to individual resident needs. Collaborate extensively with medical professionals and family members, ensuring continuity of care and advocating for residents’ well-being. Your clinical leadership will guide team members on critical issues such as symptom management and dementia care.
Key Responsibilities:
• Assess resident health to determine treatment needs
• Collaborate on care plans with health professionals
• Participate in resident and family care discussions
• Guide LTC team in best care practices
• Evaluate care trends for educational improvement
Requirements:
• At least one year of clinical experience in relevant fields
• Registered Nurse in the Extended Class with Ontario College
• Familiarity with relevant health statutes is a plus
• Leadership skills are beneficial
• Additional certifications in gerontology are assets
