Job Description

Job Title: RN Case Manager

Are you an experienced RN with Case Management experience looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered “yes" to any of these questions – this is the position for you!

Position Summary:

As the RN Case Manager, you would be responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating case management activities across the continuum of care.

Hours for this Position: Monday-Friday (40 hours/week)

More Insight of Daily Responsibilities:

  • Check eligibility as per procedure/ client specifications
  • Assess initial clinical information and develop a treatment plan and goals that meet the individual patient's/ client's present needs
  • Communicate goals and objectives to all pertinent parties
  • Consult physician advisers to determine the medical necessity and appropriateness of treatment plan goals and objective
  • Determine the cost effectiveness and medical necessity of alternate treatment
  • Negotiate with providers when necessary to reduce patient cost
Qualifications

What We Look For:

  • Clear and active RN license in the state of CA
  • 3+ years of clinical practice experience
  • Case Management Certification preferred
