Geriatric Nurse Practitioner Position

As a dedicated Nurse Practitioner, you will work collaboratively with a multidisciplinary team to assess and treat residents' health issues, prioritizing their comfort and safety. Your ability to engage residents, families, and substitute decision-makers will be essential in ensuring personalized care plans. Additionally, your leadership will foster a supportive environment for nursing staff and other healthcare professionals.
Key Responsibilities:
• Assess and manage health conditions with a focus on individualized care
• Collaborate closely with physicians and care teams
• Advocate for residents during decision-making processes
• Lead initiatives around pain management and end-of-life care
• Monitor and report on trends to improve care standards
Requirements:
• Minimum one year of nursing experience in long-term care
• Current registration with the College of Nurses of Ontario
• Knowledge of applicable healthcare acts is beneficial
• Ongoing certifications in geriatric care are assets
• Leadership knowledge or experience is considered an advantage
