We are looking for a Registered Practical Nurse to provide individualized assessments and care plans while leading care for clients. Key Responsibilities:
• Execute comprehensive client assessments and develop care plans
• Lead care coordination as a primary provider
• Collaborate with multidisciplinary teams for optimal client outcomes
• Facilitate transition of care and community connections
• Utilize evidence-based practices to tailor services
Requirements:
• Valid Certificate of Competence from College of Nurses of Ontario
• Flexible availability for 12-hour day/night shifts
• Immunization and criminal background checks required
• Current CPR certification
• Physical capability to perform nursing duties
