This key role focuses on guiding a smaller caseload, fostering long-term participant relationships, and enhancing behavioral health program development. You will collaborate with interdisciplinary teams to ensure high-quality, comprehensive care under the Medical Director's guidance. Your dual focus on patient interaction and operational efficiency allows for impactful contributions to vulnerable senior populations.
Key Responsibilities:
• Provide psychiatric evaluations and treatment collaborating with physicians
• Lead and mentor behavioral health team members
• Facilitate discharge planning from long-term care settings
• Manage care of nursing home participants through regular visits
• Drive quality improvement initiatives and clinical audits
Requirements:
• Master’s in Nursing with psychiatric emphasis required
• Certified and state licensed Psychiatric Mental Health Nurse Practitioner
• Minimum four years’ experience with elderly patients
• State CDS Certification, NPI, and DEA license
• Preferred experience in PACE model and as functional lead
