Enhance resident well-being with Age Care as a Part-Time Nurse Practitioner in Queens Garden. Provide quality care while collaborating within a person-centered environment.
This temporary part-time role at Age Care focuses on delivering comprehensive resident care two days a week. You will work collaboratively with medical professionals to assess and treat both acute and chronic illnesses. This position emphasizes supporting residents and their families through all aspects of care planning and clinical leadership.
Key Responsibilities:
• Collaborate with MD and interdisciplinary teams for care delivery
• Assess, diagnose, and prescribe treatments for residents
• Advocate for residents’ best interests in care discussions
• Lead clinical support for the long-term care team
• Identify trends in resident care for educational programming
Requirements:
• Minimum one year in geriatric or long-term care nursing
• Leadership knowledge is an asset
• Certifications in gerontology or palliative care are preferred
• Current registration with the College of Nurses of Ontario
• Knowledge of relevant health acts is advantageous
